Because they, like many other things, are part of the anachronistic pay
structure that we, as a profession, appear to be clinging onto. I cannot
remember the last time that I picked up any social or family conditions
that I didn't know about or the MW or HV missed that I could actually do
something about (I accept that parts of the country may be otherwise).
As I become older and less apostolic however, I quite enjoy surprising
the happy family (if they are at home) and spending a few minutes with
patients that don't want me to change their world for them.
