Please don't ask for a 'watertight' solution to confidentiality;
such things do not exist in the IT world any more than they do in the
world of paper based records. That said, it is feasible and
affordable to devise IT based systems which are both much more
secure, and much more accessible, than any paper based system. It is
NOT necessary to have the data held centrally for this to be done;
indeed, there are positive benefits in hoding the data distributed.
