Bit late in on this one becos off on hols. But yes, this is a major
difference between service industries in general and the socialist
experiment which is the NHS. Such a major difference that one would
imagine it would be shatteringly clear even to the ex-tea boys who make up
NHS management these days. The fact that such a massive difference is not
immediately (or eventually) apparent to those gents like Roy Lilley
suggests some underlying problem such as sub-clinical congenital
hypothyroidism.

That is precisely what we have to do. Very sad but then, once we get over
the emotional upset we shall see that the patients get exactly what they
pay for (did I hear shouts of "Crap"? Yes, that is exactly what the
pittance paid to NHS GPs buys these days) and once we all stop trying to
save the world then society as a whole can settle down to the difficult but
worthwhile task of constructing a health service which will provide what
the funds allow.
