A few years ago one of my partners decided to vet requests for house
calls at the time that they were made. The result was, and remains to
be, a large reduction in his, and now my own visiting list. When I
joined the practice 17yrs ago it was not unusual to have >6 calls per
day: now I expect an average of <2/day. The downside is the probable
intrusion into a consultation but, I recommend the policy. The patient
with you (if your consultation is disturbed) will become aware of the
demands on your time/expertise and may well alter their behaviour as a
result. The caller has immediate access to what he/she has asked for -
which is the ideal situation as far as "defusing" is concerned and,
makes the problem so much easier to deal with if the caller is in
"relief" mode rather than "fighting the system".
