The thing about general practice is that each consultation has its own
expected utilities and that the importance of process/outcome is largely
suggested by the patient. It has been suggested by some (D. Black) that
general practice consists of 90% process but that is a statistical
comment rather than a value judgement.
Outcome in GP is a many-headed beast, ranging from the simple fact of
having made an appointment (to please a relative or boss) to saving a
life (ocassionally). In my view it is this massive spread of outcome
expectation Versus limited resources that is causing problems in the NHS
generally. I believe the time has come (?1950) when that spread has to
be narrowed to preserve the ability to cope with the process.
