you need to develop and practice set patter eg

For things like those infernal private letters beloved of istitutions
but not one I list as necessary like safe to fly at 32 weeks pregnant

THe nurse is much more practised at doing these these days and I could
have seen someone who was ill amd needed the skills of a mere doctor

Well i suggest you negotiate with your fellow patients in here (they
usually collar you in the waiting room) I don't mind what order i see
people as long as I am not waiting for people who DNA
I am sure we do all have balls but the trouble like parenting is being
consistent and giving the patient some responsibility.
I never tell people to stop anything. I ask them if why they smoke etc.
I check that they are fully aware of the risks and then say 'well you
are a responsible individual, so if and when you decide to give up we
are here to help.' They have to decide - it is their body.
