Think of IBM, Microsfot, ICI, Virgin, BodyShop or any other
organisation you can think of to see what I mean.
All the structures being built right now to take over the management of
Primary Care Firms by April next year are based on loosely defined
criteria. The enthusiasts and activists are naturally taking the lead.
Some GPs in particular have had experience in these matters and they
tend to lead to setting of management structures. The other parties
that the White Paper wants to engage are in a muddle or on a different
planet.
Add to that the fact that there are no identifiable mechanisms to
select representatives from various sections and the question of
democracy goes out of the window. For example, how on earth do you
select representatives from the community a particular Firm serves?
If we were to insist that Primary care Firms' management structures be
elected by universal suffrage then the whole exercise becomes a
logistical nightmare if not utterly unworkable.
Some GP structures are even being built on totally and blatantly
undemocratic principles, for example, votes are allocated according
to List size! This practice must be resisted and overturned where it
is exercised.
PCGs are business organisations that are composed of both shareholders
(the GPs) and stakeholders (everybody else). This arrangement would
make it impossible to exercise standard business processes in
establishing governing boards and other management structures.
Either everybody in the Primary Care Firm becomes a shareholder, which
is not practical or workable; or everybody becomes a stakeholder,
which will entail stripping the GP shareholders of their assets and
compensating them for it.
If we do that, then the stakeholders' General Meeting can bestow
executive powers in any way it wishes as well as determining the
constitution of the Firm amongst other matters.
