Sorry, but I don't think you have grasped who is to say what to whom.
You make it sound as though somebody other than the patients has
decided what is to be had....
We, individually or as PCGs (and collective action has much to
recommend it over lone action) can tell patients what the amount they
have decided to pay (collectively) wil buy them as health care.
We can tell them, and surely should since some of the money will come
to us<g> that they should pay more.
We can offer advice on what healhcare they might best buy, in addition
to that which they have already decided to buy....
And if the process is scrutinised by the citizens and their
representatives to see that we are giving good advice, good. (We can
advise them what we need in order to give better advice, as well.)
Who could do it better? Nobody.
The key point here is that none of the numbers involved are to be taken
as given or constant. The loop can be closed.
