I hate to bring up the subject of fundholding, but our trusts really pay
attention after we staged a small firepower demonstration in 1993 (we
responded to a unilateral action by the trust with a unilateral action of
our own).
As fundholders, we found it worked much better if we banded together in
groups and subverted the clinicians, before specifying anything in
contracts. The system runs pretty smoothly now - it is one of the reasons
why I think PCGs will work in many areas, since they will encourage the
situation I have described.
