If you are referring to clinical governance, David, the person with
responsibility will be the PCG chief exec, who in all likelihood will be an
NHS manager.
Doctors on the board of a PCG will have responsibility, but only in a
corporate sense. For example, a level one PCG isn't remarkably different
from the commissioning groups which have existed for several years - and
which fought to get responsibility.
As for funding, well, at my calculation, three quid a head buys you a staff
of six or eight, including a chief exec on 45k. At level four, back of the
envelope calculation says funding will be over six quid a head, maybe
higher.
A huge amount of the arguments on this subject which centre on 'blame' are
merely a recycle of the arguments around fundholding - where a section of
the GP community became obsessed with the idea that once a practice became a
fundholder it would get 'blamed' every time a partner farted twice. Can we
have some new arguments please?
