Interesting.
If a lady who has breast cancer has unprotected sexual intercourse
mid-cycle (USImc) what would we do? Is there any reason to think that
the risk of a pregnancy, with high levles of ovarian hormones for a
couple of weeks to several months would be less than that of a quick
blip of PC4. I doubt it.
IUCD I suppose, if the tumour has been definitely diagnosed, but not
all women are keen on them.
If a lady with a breast lump (assuming that we are not examining them
to pick out fungating cancers, but something a little more subtle) has
had USIMc are we going to refer her for asseeement by whatever the
current favourite is - mammography +/or ultrasound, plus a needle
biopsy - and have that completed within the 72 hour window for PC4?
Given the suggested relationship between bipsy of breast tumours and
menstrual cycle stage, it may not even be the best time to investigate
the lump, and I know of no breast service set up to manage them in such
timely fashion.
Surely again the risk is in favour of giving the PC4.
So, the detection of breast cancer is independent of the provision of
contraception, and should not be linked together.
This seems to me to be a general approach inflicted on women, BTW, and
hopefully will become less common.
