I don't think anyone would doubt that for one minute.
However, the doctor has a 'license' to over ride what the 'book' says.
We sometimes prescribe drugs for indications that the makers did not
include in the product data sheet.
It is up to that doctor to defend the decision should it come to
litigation if and when harm can be shown to have occurred as a result
of such a decision.
