What literature exists is sparse and generally unhelpful by virtue
of stating the obvious. However, it would be well worth reading two
papers from the oft-maligned British Journal of General Practice,
November 1996 issue. Harris and Dajda (pps 649-53) describe how bad
things are while Zermansky (pps 643-47) offers very useful
advice on a 'model' system which was tried.
