I wouldn't go down this route if what unless you want general modem sharing rather than just email and web access.
I'd look to an email gateway which will allow email to the desktop. I use VPOP3 on my network at home (www.pscs.co.uk) £40 for the 10 user version. You can do something similar with NHSNet for £3,000 - £4,000.
I'd use a proxy server to allow web access. This will allow multiple users to browse the web at the same time over a single modem connection. (can be slow if you get more than a couple of people on a dial up link). pscs have a simple freeware product VSOCKS or look at www.davecentral.com where there a lots of products listed.
If you are going to connect the network with you clinical system to the Internet you need to consider the security issues.
