I remember writing a letter very like that back in 1989 when I was doing
locums and the new "contract" was on its way. I sent it to Reichsfuhrer
Clarke (pardon the omission of the umlaut) which was a total waste of
paper. Things have not changed all that much in the last 8 or 9 years.
Why does this go on? First, the NHS cannot afford the care we would like to
give. I agree with all those who said say no, limit availability, be tough
etc etc and I changed my own practice along those lines but it is a bugger
reining in the caring instinct.
Second, we cannot afford the care we would like to give--the cost being
personal development, family life and sanity.
It really is them or us. And if we conk out then they don't have us any
more so they benefit by us putting us first. But it takes something awful
to happen to you to make you see that---hope you are able to change things
in your practice before you burn out or get depression or divorced or
whatever.
