In Devon at any rate, our GMSC rep and the LMCs are predominantly in
favour of the LMC policy that every GP who wishes to should be
afforded the chance to be in an out of hours co-op.
The LMC members who abstained or voted against (all this is a matter
of record in the GP if not public domain, in theory) where those who
were accepting the democratic will of their constituents, in at
least one case those who are in a co-op, but anticipate that if the
benefits of this are extended to their out of town colleagues they
will have to pay more or work more.
These committees reflect the nature of their constituents, which is
in an excessive number of cases selfish, mean and shortsighted.
One of the reforms which you can call for which would oppose this
and reduce the strains on PCGs is to aim for a reduction in the
capitation elements and an increase inthe site-relate elements of GP
remuneration, thus making the business less competitive and easing
co-operation.
Increased funding does seem to be needed to ease the passage of
rural GPs into collaborative arrangements, compared to the £1400 per
year needed to set up the urban solutions.
