A possible 'solution' to this kind of verbal tirade is just to sit
there and say nothing. And I mean *nothing*. Give no
feedback/cues/etc. After a while they tend to get bored and shut up.
Then start the consultation all over again.
Shouting and giving verbal abuse back will only make the situation
worse.

I find this works the best. If aggressive people see you are not really
interested in their outburst and it has no effect then they do shut up. My
toddler soon learnt this one. Pity we can't put our patients in a quiet
corner as "time out".
