We withhold the outgoing number for calls made from the surgery main
number - but not through paranoia. On day one of calling number being
available, we started getting calls from patients saying "you rang me and I
am returning your call". As there are up to 50 people based here, finding
out who made the call was impossible, so to stem the tide I got BT to switch
it off.

I have a called ID system on my phone, and look at it before deciding
to answer the calls. If I don't the answer machine cuts in, but the
choice is mine.
Withheld numbers are often not answered as they tend to be nuisance
calls - with one exception a GP friend who for paranoid reasons ( are
you listening Dr G ? ) withholds the number by default.
On a similar note my new mobile allows me to withhold my caller ID if
i wish, with my old one I didn't have the choice.
