When I started as a GP principal, I had a patient (now deceased) who was
told by his previous, long retired GP that his isoprenaline inhaler
(Medihaler Iso-Forte) was useless when half-full, and was using two inhalers
a week for some years before I met him.
His inhaler technique was fortunately so bad that his dog got much more of
the isoprenaline than he did, but he was ineducable despite several long
consultations explaining why he should use other agents through spacer
devices. I even had a joint consultation with him and his sons to explain
how important it was.
I learned from this that there are some things that you can't change, and
I've tried to avoid beating my head against a brick wall since then.
