Thanks to all those who answered my query regrading matrix sampling.
I received only a few replies with information or advice, and many more
requesting copies of the information I receive! Perhaps this is an
under-published area of research, especially in light of the growing
application of multilevel analyses which efficiently deal with such data
structure? This is further reflected by one respondents comments. The
comments I received were:

Jon Rasbash <[log in to unmask]> (Multilevel Models Project) suggested I
look at the PhD by Geoff Woodhouse (Institute of Education) on analytical
solutions to missing data in multilevel models, and the PhD by Dougal
Hutchinson (NFER) on simulation based (bootstrap) techniques for missing
data in multilevel models.
Ron Spiro <[log in to unmask]> was also interested in learning more about
matrix sampling, and he submitted the following:

D. Kaplan, The impact of BIB spiraling-induced missing data patterns on

Steve Gregorich <[log in to unmask]> suggested an article by John
Graham et al. 1995 (?) in Multivariate Behavioral Research which discusses
the "3 form design". However, I have yet to trace this. If anyone finds
it first, could they please let me (or the list) know.
Jane Hutton <[log in to unmask]> also advised me to contact a
colleague locally in London for help with study design - Dr Rosemary
Bailey, at the Queen Mary Westfield College (Barts & The Royal London),
although I do not have any further contact details.
