GP-based anticlog clinics are a live issue at the moment - there was a piece in a recent BMJ, not to mention this week's issue of Pulse.
Devon GPs have been paid a certain sum of money per patient for the last couple of years as the hospital clinics wind down and we have been reasonably happy to pick up the pieces.
What has made that job a lot easier has been one of the computerised programs which determines dosage schedules and review intervals according to an algorithm published in the BMJ quite a while back. As a result our audited figures have risen from a measly 28% of patients within therapeutic range to a much more creditable 45%, and still going strong.
The program we use is called 'INRstar' - it's a Windows-based application and costs #250 per user-practice. I have to admit to a certain financial interest in the package, and since I know GP-UK is not a forum for advertising, I'll call a halt at this point!
