Bearing in mind that most of the clinical features also occur in more
common illnesses and there is no diagnostic test, you are very likely
right, but any cases which are missed are likely to be milder than
those diagnosed. In particular it is very unlikely that any fatal
cases go unrecorded.
Maybe this is another of those diseases which are severe and life
threatening in hospital, but often seen in much milder form in the
community, e.g. croup or erythema multiforme?
