We (Cambridge) also have a separate Exam Moodle (we call it Assessment Moodle - as it can be used for exams and courseworks etc - though it is primarily used for exams).
A few things:
- It does not integrate with our other Moodle(s)
- It does not integrate with any of our records or timetabling systems (though none of our Moodles do)
- Content on there is entirely controlled by the Exams & IT/Digi Edu systems teams. Though content is co-designed between the digi edu systems team and the department
- Enrolments are done, by and large manually (sometimes with the help of csvs) - this again is the same for all our Moodles
- we are hosted by Catalyst
- we use both the quiz and the assignment tools
- we don't support (yet) marking, grading or feedback
We are currently going through an exercise to determine if a) we buy in another system (such as Inspera, WiseFlow, DigiExam, etc), b) we expand/improve the Assessment Moodle offering (lockdown browsers, more automation - within reason/limits etc) c) we do a combination (most likely)

We do have a separate instance of Moodle specifically for Exams. There is no direct integration from our Exams instance to our regular teaching Moodle instance but they both use integrations with our student records system and timetabling system to automate the process of Moodle space creation and enrolments.
The exam instance has a restricted set of Moodle activities and plugins enabled and slightly different colour scheme theme to help distinguish it.
Within our exam timetable system, we have created a virtual exam room called "Moodle" to which exams and students can be assigned. Any exam assigned to Moodle triggers an Exam Moodle space to be created automatically with staff given immediate access. Students are enrolled and assigned delayed access (1 day prior to the exam). We also pull in any ILSP data regarding extra time and rest breaks for students and add them to appropriate groups within their Exam space.
Staff are responsible for setting up the appropriate exam activities and configuring the ILSP overrides where appropriate.
A link to the Moodle space is then surfaced to students on their exam timetable to enable direct access to the exam from their timetable.

I am interested to hear from any institutions who may have a separate Moodle instance which is solely for online exam hosting and which may be integrated back into the core Moodle VLE (or other VLE) via LTI or APIs.
