Thank you all (Janet, David, Mike, Mary, Naren, John) for all the various inputs. I think it's quite clear that if this item is related to Rudyard Kipling himself, it must be from either his time in the Indian subcontinent or South Africa. I shall try to look more in that direction.
And to answer a question / comment that came up a couple of times, the ivory was provided by a boar.
