Peter,
All we could do is tell you what others with those diagnosis find useful, It
would be unethical to go further because your mix is different from our mix.
Furthermore, diagnosis tells us what systems are impacted but not how. It is
as though you have asked us what you need for a missing limb but you haven't
told us if that is an arm or a leg.
Here too in the US. Our courts have mostly decided that employees need not
know what might accommodate to their needs and it's acceptable to ask for an
accessibility assessment as an accommodation. This could cover more than a
workstation assessment.
On the other hand, the more you know about what works for you the better.
Here in the US I would go to my local independent living center which has a
tech person first. They are free and would help me line up my ducks before I
go public with a request. In school, or with a brand new impairment it is ok
to not know exactly what you need/want but in a job???
I would at least say that I have used x, y, z but I haven't had a full
assessment in a while so I don't know what is new.
Good luck

Would tend to be the same in Canada where the company would do an assessment
and let you know what they have and what they can get you.
