I hope you don’t mind me picking the POCT collectives brain but we are after an explanation for an incident at the hospital:
We have had a case recently where we had ketones HI > 7 mmol/L reported on a patient. The diabetes specialist nurse thought that this wasn't in keeping with the clinical condition and so went and asked the nurse to repeat - again a reading of HI > 7mmol/L was obtained. The diabetes nurse then repeated on the same arm and got 1.4, which correlated better with the clinical condition.
The diabetes specialist nurse stated that when he watched the original nurse there did appear to be squeezing of the area - something we are very clear about NOT doing in training; however I would have thought this would have resulted in an artificially low ketone reading due to the interstitial fluid dilution.
Does anyone have any other possible explanation? The company keep mentioning dilution effects of squeezing but this doesn’t explain a high ketone result.
