Really useful discussion here, thanks everyone. We are currently running rapid Cepheid in the microbiology lab alongside two m2000s. Seems to work well by all accounts and the only issue is policing the ordering of the rapid test use.



We have the flu/COVID testing on our radar and will shortly be discussing what the model should look like - POCT ED/Lab combination service but nothing decided yet. Seems there are regional and national discussions and not helpful to see reports of central government purchasing large quantities of tests for COVID on devices that are not tried and tested in the clinical setting for large scale testing. Not sure the government leads have learned anything about diagnostics during this pandemic and they seem intent on jumping in, both feet first, to secure new tests.



It is possible that this will be centrally managed and services will be told what to use, and supply controlled in the same way lab allocations have been. One thing is for sure - devices with complex processes will never be a success deployed in busy departments with competing pressures on resources.

We're using the Menarini - 30 mins to get a result. We run it from our hospital based lab (yes - think these type of instruments do pose a problem if you are trying to use them with a large pool of  HCA/nursing staff). We provide the testing 24/7. Key thing has been working with clinical site teams to ensure that priorities for fast track testing are clearly understood - order comms has been a valuable tool in "demand management" If not fast track, samples get transferred to our hub lab (again 24/7) where average TAT from receipt is around 5 hours



Capacity of Menarini is around 50 tests per day (obviously works best if you can have samples arriving adequately spaced out!!)

This is really helpful.

Are these connected up to IT? Just wondering how you are collecting the results for PHE,etc.



I have seen the brochure for the DNA nudge and although again the loading of a sample seems reasonably straightforward the maintenance looks complex.

There is a 1page flowchart that users should follow to perform a Health check to test the Nudgebox pressure and thermal subsystems at the start of a day and after every 10 tests. This takes 10mins and is complex.

Our nursing staff would really struggle with this.



If anyone has first-hand experience of the DNA nudge I would be really grateful if you can feed back with regards to operation, maintenance and collection of results.



This would be really helpful as we are trying to plan flu and COVID. We also have a Cepheid in our Respiratory dept but don't know what cartridge allocation we are going to get which makes planning difficult.

We have been running a POCT service using the DRW SAMBA devices since mid-may. We now have 5 services running at 5 different hospitals.

The service is run out of A&E. It is staffed 24/7 by POCT associate practitioners - as the devices are complex and the service is very busy - removing nurses/HCAs from patient care activity to run this would not have been the right decision.



However locating them in A&E and considering the approach to testing along a POCT operational model has allowed us to provide an average turnaround time from swab to result of 2hr 35min. Which I don’t believe has been possible using these devices when they are located in the laboratory.



The loading of a sample is simple. The troubleshooting is not as simple and requires a good SOP to follow through.



1 test is 98min analytical time for a negative. You can only load 1 sample at a time.

So they come in banks of devices.

In between tests the time to clean down and load a sample is approx. 5min for an experienced operator.



So in reality... the company sell them with expectation of running 12 a day per machine.

You can push this to 13 a day in my experience.

Happy to discuss further if you would like to know more.





I do not have any experience of the DNA nudge, I have only seen the presentations circulated to procurement teams nationally.

I understand they implemented at charing cross and imperial.

With some verification work done at other sites.
