I have been working on EU projects for some time now, and in particular as an 'Independent Ethics Mentor' consultant for a large European Commission-funded project, a role that focuses on compliance with ethical standards and data protection regulations most notably GDPR.
I am writing to confirm that what Ian has aid below is absolutely correct. I also wanted to offer that this website (from an AHRC funded project on museum evaluation) has an example of a GDPR compliant consent form (and other free downloadable resources): https://www.practicalevaluation.tips/resources
This example is longer than you will want yours to be, because it is meant for an online survey form. But it should give you an idea of the kind of ground you need to cover.

As far as I understand, the GDPR regs apply to paper forms just as much as they do to online processes, so going low-tech is not a get out clause. If you're using cards to collect email addresses you still need to tell people why they're being asked for an email, what it will be used for, and request that they opt-in to this collection and usage of data. A simple step would be to add a tick box to the cards to say something like "I consent to you collecting my email address and using it to contact me for purpose X"
Should add, I am not a GDPR expert
