Fascinating discussion and something we tried to grapple within some of our work
Largely agree with Rod's views on this also
In case useful, we tried to explain a randomised trial simply for an audience that included the public (see p. 8 of https://priorityresearch.ie/wp-content/uploads/2017/04/192791-hrb-report-online-final.pdf and here https://www.ncbi.nlm.nih.gov/pubmed/29490702). I recall many discussion about 'what is a health care intervention'. Discussions included anti-fungal paint for kitchens and impact on asthma, speed bumps, architectural designed space for wellbeing etc.
We've also had a go at this for a public involvmengt project we have almost finished 'The People's Trial'. Video here if interested https://thepeoplestrial.ie/about-us/
All with caveat of course that others may describe differently...
Happy new year
