All good tips from those responding - I recall the calibration line being a problem on many sites ignoring the cleaning processes we had put in place. It was quite obvious once you took the tray out to look. Just be sure you don't use anything too abrasive to clean it as it could rub off. Probably not the case now. Other problems were excess urine splashing inside the analyser onto the optics, which you cannot clean yourself. Once dry, there would be no way of observing it there.
And lastly, was this the patient's urine? I ask because we had a patient whose friend produced a urine for her (presumably she had taken something illicit) and didn't realise a pregnancy test would be automatically performed on the unit due to her age and what she was being investigated for. Turned out her friend was pregnant and didn't know. It caused us a headache as it took a while to investigate as the patient categorically denied she could be pregnant but didn't own up.

Thank you Kate,
This is something I hadn’t considered. We do not have a maintenance schedule in place for this machine.
I can’t replicate the error, and don’t have the original sample to try.
Interfering substances are highly unlikely (but never impossible) in this case.
User error has not yet been ruled out, but neither has instrument error - so I have sidelined the device pending outcome of the investigation.
The ward involved followed the correct procedure as it was an unexpected result- a sample was sent to the lab for confirmation and POCT informed.
Thank you everyone for your input.

The analyser uses optics to look at the calibration bar to determine the line on the HCG cassette. This should be checked regularly and can be washed under the tap and dried properly prior to reinserting the test tray.
