Always ask! You do have to have permission to post anything on Insta, right? Instagram's Ts and Cs make you responsible for any content you post...
As far as I understand it, Insta doesn't provide you directly with the tools to regram within the app (unless you're using a paid formal partner tool like Sprout or Hootsuite), so any reuse is basically equivalent to copying and reusing (unlike, for example, retweeting where it's something you're signing up for when you post content).
99% of people say yes when we ask, but not all.
