Unfortunately, Ali, there seem to be major questions about the feasibility of trying to provide high quality midwifery with continuity of care in any sort of set up, public, private, or something in between. I find it incredibly ironic that 'continuity' is now being presented as a shiny new thing for supporting people with chronic disease, when many parts of the health service seems to be determinedly undermining all policy efforts to make it happen in maternity care - where it was absolutely the norm until mass hospitalisation, and for which, uniquely, we have rock solid evidence of effectiveness. I cant help but think that the particular difference in maternity care is that it is about women...
