Hi Yusuf
No. Our community midwives rotate back into the hospital and therefore the meters return to the hospital base where they can be tested in this way. That's not to say we have this set up exactly the way we would like! I would not advocate taking blood unnecessarily in the community from babies and there are all sorts of ethical barriers to doing this in the hospital or outside. The correct consent should be obtained. If the blood is redundant due to other tests being performed then this has always been acceptable in terms of use to verify results with the lab method. Still a bit of a grey area though.
However, some TCB measurements indicate the baby has to be brought back into hospital and therefore would need lab measurement or a measured bilirubin on a blood gas analyser if the method is validated for use.
It is complex and should be a protocol discussed and approved by clinicians from the lab and the neonatal team.
Good question! Now my head hurts

Hi Tony,
Would you recommend this approach for community TCBs?

Hi Mary
I don't think there is a way of providing traditional approach to EQA with TCBs but am happy to be put right if members know of a scheme.
Regular comparison with the lab result would be an approach you could take. Bloods may well be being sent to the lab for other investigations at the same time so a lab bilirubin can be requested and compared in relation to what your equipment validation established for the performance criteria of TCB vs Lab.

Dear All,
Does anyone know if there is an EQA scheme for the transcutaneous bilimeters? If there is not, what do others do to satisfy the standard around EQA?
Kind regards,
Mary
