I have been approached by many confused members regarding the advice to delay cord clamping for 1-3 minutes which is not based on any recent evidence at all and is a retrograde step in the battle for babies to receive their cord blood.
The latest evidence by Ola Andersson 2018 shows that clamping earlier than three minutes can lead to anaemia and that 1 min clamping is now early clamping. NICE used the 2013 systematic review to recommend 1-5 minutes. Almost 6 years later the RCM is using this review to advise 1-3 minutes. In the light of the most recent evidence this is damaging to babies and there is great consternation amongst the campaigners, experts, lay people and midwives.
The one minute recommendation also gives the green light to the cord blood companies, particularly the public banks who do not offer informed choice to parents and do not advertise the benefits of dcc/occ on their websites (they encourage the largest sample possible) to encourage parents to store/donate their babies blood without knowing that consequences of their actions can adversely affect their babies development.
The RCM have been well placed to lead the way in bringing evidence into practice but in publishing this guidance with no input from experts have seriously undermined the research of the researchers and the effects of the global campaigners who are battling for evidence based practice. We do not have any sound evidence t say when it is safe to cut a pulsating cord.
The guidance needs changing and the timing of 1-3 minutes needs removing.
