For many years we at the Economic Botany Collection at Kew have used a modified version of the British Museum form EE1, downloadable from: http://www.britishmuseum.org/about_us/departments/conservation_and_science/facilities_and_services/collection_scientific_study.aspx
It covers the key criteria and questions very well; we have also found that having to fill in a form is a useful gateway for serious enquiries.
For biological material, all samples from Kew for non-commercial use also require a Material Transfer Agreement, which ensures compliance with the Convention on Biological Diversity. The Kew version (many others are online) can be downloaded at:

Subject: Re: Destructive techniques policies
Hi,
There was another article in the NatSCA journal a few years back: https://www.natsca.org/sites/default/files/publications/JoNSC%20Vol2-1.PDF<https://emea01.safelinks.protection.outlook.com/?url=https%3A%2F%2Fwww.natsca.org%2Fsites%2Fdefault%2Ffiles%2Fpublications%2FJoNSC%2520Vol2-1.PDF&data=02%7C01%7Cm.nesbitt%40KEW.ORG%7Cb84591f8c2ec4df50be008d632afcabe%7C44892b45194046119753a4b3be4ea7fe%7C0%7C0%7C636752129007611826&sdata=xbdj5mNLOzwr5R45fgO%2F4I%2BfZEsxYYQbybnLPy9I9tc%3D&reserved=0>
In the appendix is an example of a DS form.
I’ve also attached the NHM’s Destructive and Invasive form. There are some details on the NHM’s website including somethings to take into consideration: http://www.nhm.ac.uk/our-science/collections/accessing-collections/loans.html<https://emea01.safelinks.protection.outlook.com/?url=http%3A%2F%2Fwww.nhm.ac.uk%2Four-science%2Fcollections%2Faccessing-collections%2Floans.html&data=02%7C01%7Cm.nesbitt%40KEW.ORG%7Cb84591f8c2ec4df50be008d632afcabe%7C44892b45194046119753a4b3be4ea7fe%7C0%7C0%7C636752129007611826&sdata=kKz7Uox39Y58NvQSJ75QPqns2355hDgjvTMCDRuoKRU%3D&reserved=0>
One thing to consider is a technique which itself may not be destructive (e.g. CT scanning), but the process of mounting it could damage the specimen.
Best wishes,
Emma

Subject: Destructive techniques policies
Hello,
Would anyone be willing to share their destructive techniques policy/paperwork with me please? We’ve had a couple of enquiries and could do with writing one. Any help would be greatly appreciated.
Thanks very much
