For many years we at the Economic Botany Collection at Kew have used a modified version of the British Museum form EE1, downloadable from: http://www.britishmuseum.org/about_us/departments/conservation_and_science/facilities_and_services/collection_scientific_study.aspx

It covers the key criteria and questions very well; we have also found that having to fill in a form is a useful gateway for serious enquiries.

For biological material, all samples from Kew for non-commercial use also require a Material Transfer Agreement, which ensures compliance with the Convention on Biological Diversity. The Kew version (many others are online) can be downloaded at:

One thing to consider is a technique which itself may not be destructive (e.g. CT scanning), but the process of mounting it could damage the specimen.
