We use a Google spreadsheet and forms to help us manage some of these elements. I confess that I am not the mastermind behind it (I can put you in touch with them).
The system manages all of our requests for teaching from academics via a directory of skills and outcomes. The directory also maps the skills to our online resources which inform our e-learning team to embed the appropriate resources into Blackboard as part of our blended approach.
Via a form all of us librarians input all the data including dates and times and staff involved.
The spreadsheet collects feedback from the library staff member that delivers it about how it went and actual student numbers attending and from the academic staff member that requested it.
Overall it has taken a little bit of refining and we are still working some aspects of the data that it collects.

Subject: Keeping track of IL teaching stats, coverage and feedback in one place
Hi All
Has anyone come up with a satisfactory way of, in one place, collecting:
- teaching stats for SCONUL (ie. hours delivered by library staff, hours received by users, hours delivered to researchers)
- keeping track of what kind of learning interventions or specific learning objectives each cohort has been taught throughout each level of study
- capturing feedback from the learners
Some kind of online tool or unified spreadsheet that enables this all in one place would be amazing.
I will of course share anything that's shared with me (with permission of the originator).
