Project background:
Healthcare organisational boundaries are known to be key sites of risk and uncertainty, with approximately 70% of serious errors in hospitals occurring during initial admission, intra-hospital transfer and discharge home. Medication errors are a leading cause of avoidable harm to patients, particularly during transitions of care. The most common intervention used to reduce medication errors during transitions is ‘medicines reconciliation’, which involves healthcare professionals working to ensure that every patient has a complete and accurate medication list at every point in their care. However, making medicines reconciliation reliable has proved very difficult due to the complex nature of clinical settings and professionals having limited information on how care is delivered across organisational boundaries. There is therefore a need to develop new approaches to improving medicines reconciliation that are sensitive to the complexity and diversity of healthcare contexts and boundaries, and which effectively involve clinical teams in the improvement process.
The aim of this PhD studentship is to understand medicines reconciliation processes and practices across diverse healthcare organisational contexts and boundaries using the innovative research method of video reflexive ethnography (VRE). The final design of the study will be determined by the student working with their supervisors in Year 1, but the broad design is a multi-site video reflexive ethnography of key inter-professional safety practices during medicines reconciliation at admission, transfer and discharge in at least two hospitals and a sample of linked general practices. During the course of the studentship, particular attention will be paid to understanding how best to employ VRE as an improvement methodology across different healthcare settings and organisational boundaries.
This project will contribute new insights into medicines reconciliation safety and the application of VRE as an in-situ, collaborative improvement methodology for wider application in research and practice.

Funded by The Healthcare Improvement Studies (THIS) Institute, this 3-year PhD studentship offers a new training opportunity for an outstanding social scientist to carry out an extended supervised period of research and contribute to the field of improvement studies early in their career. The studentship will provide an annual tax-free stipend of £17,500, rising to £18,566 in the final year. Full coverage of UK or EU tuition fees and a conference/travel and research costs allowance are also included. The student will be supported by social and clinical scientists and have membership of THIS Institute’s Community of Scholars.
Requirements:
Applicants should have a good Honours degree (first-class or upper-second-class Honours) in an area related to the proposed field of study. Additionally, you must already possess, or expect to receive by Autumn 2018, a Master’s-level qualification in anthropology, sociology, science and technology studies, geography, or a related discipline.
