It's not just Medline abstracts that are an issue. I find that in Embase abstracts there is a colon and space after the heading but there's no space before the heading.
I'm sure that the abstracts would not be designed to display that way in their native interface so it has to be an issue with the way the data is transferred over.
perhaps NICE could be a little more insistent about this as it's really not very satisfactory.
I almost feel inclined to add a rider to each set of results. "We apologise for the odd spacing that occurs in these results. This is caused by the database providers (ProQuest and Wolters Kluwer - or wherever NICE get the databases from) and is beyond our control."
Maybe, if we all did this, the bad publicity might spur the providers to take the issue seriously.

I don't know about others but the lack of spaces in Medline abstracts downloaded from HDAS has long been a niggle of mine, where there is no space after the abstract subheadings. It looks very unprofessional in search reports. I contacted the NICE help desk about this. Response below.
"Our developer has looked into this for you and unfortunately, this is an issue with Medline. They provide HDAS with the abstracts, and they control the way it displays. We cannot control how it is formatted after it comes through the api to HDAS."
So I just have to put up with it or spend a lot of time inserting the spaces. Maybe set up a Find and Replace process.
