I wouldn't put in the report. I have also threatened to discontinue a consultation because a service users partner became verbally aggressive. I politely reminded him that he was there at my discretion to support his partner, however if he carried on being aggressive that I would discontinue the consultation and inform the employees manager the reasons why.

Subject: Re: [OCC-HEALTH] Support from friend/colleague at OH appt
In my opinion what goes on in the OH consultation is down to the OH nurse. I make it clear however that the friend is there for support and not to comment. I also advise that where the conversation will remain private as far as I am concerned I cannot speak for the ‘friend’. I do point out that no conversations should be repeated outside the room. I would put in my notes that a colleague friend was present but not in the report as why is that necessary ?
If however the ‘friend’ became vocal they would be asked to leave. If the employee would not continue without the colleague present I would inform management why I could not continue with the assessment.

The colleague did take notes, but assured me it was as an aide memoire for the employee being seen. Again, a good idea as it is a mental health related case (work related).

As this is not my issue, my initial thoughts are that I have no need to acknowledge that a friend/colleague was present. if the employer finds out, then it is for them to address this issue with the employees.
