The appropriate doctor would be an occupational physician, or a GP who is willing to sign off on a pro-forma with specific statements covering the issues I outlined in my previous post.
I suggest that you reserve consultants from another medical specialty for cases where the employee has a specific medical problem related to that specialty.
The underlying medical assessment could certainly be completed by a competent OHA. Given the potential for incoming flak in the event of a subsequent problem, I suggest that the final signature on the medical fitness should be that of the occupational physician, unless the OHA is working within a police or other armed force.
There isn't much point in carrying a firearm if you aren't permitted to use it, so I don't see any distinction between these. Each renewal assessment should involve the same examination and should ask the same questions relating to current symptoms. The only possible area of difference would be in relation to the past medical history. This was obtained on the initial assessment, so questions at the time of renewal could centre on changes since the previous declaration, or progression of previously declared symptoms. In practice, it is probably simpler to have a single procedure for both initial and review assessments.
