I received very helpful feedback, together with a couple of requests to share the feedback.
1. LibAnswers by Springshare is highly recommended by 3 library services. Reasons were - easy to work with, customisable, works in multi-site services, good reporting functionality, helpful provider who gave assistance with set up.
2. One library recommended TopDesk if looking for use by multiple departments, IT, HR, library etc.
3. One library manages fine with Microsoft forms in Office 365. Reporting per site possible. Reports - standard reporting function for graphic reports or export to CSV for raw data.
