As attachments may not work I'm pasting the text of our form here. It is important to bear in mind that some PhD students and academics may actually not hold any rights for the data they are collecting for their research and may need to ask their university or funding body for permission.
We also have a policy document and an agreed retention of seven years. Forms are scanned and kept in annual files in our records management system to facilitate disposal in the future.
Copyright for non-staff catalogue data contributors
As The National Archives is a government department all the material we create (including catalogue data) is subject to Crown copyright. We require all of our non-staff catalogue contributors to assign to the Crown the rights in any material they have created for us, so that it can be made available via the catalogue and freely reused. All non-staff contributing data are required to sign this form before they contribute.
Please, sign and date this agreement and return it to your project supervisor who will forward it to the Head of Cataloguing.
I hereby assign to the Controller of Her Majesty's Stationery Office on behalf of the Crown, for the term of the rights, the whole of any copyright, database right or other intellectual property rights that will or may subsist in material created by me in the course of my cataloguing contribution at The National Archives. I also hereby waive any and all moral rights in such material.

I've sent separately the forms that I use for copyright to Nicola Waddington. However as this may be of interest to others on the listserv I thought I'd mention that on page 313-315 of Tim Padfield's book 'Copyright for Archivists and Records Managers' (Facet Publishing 2015) there is a model copyright assignment to a record office. So this would be a good starting place for drafting a copyright assignment form.
I would recommend this book as an excellent source of information on copyright. It's got some great flow charts to help you work out if something is still in copyright or not.
