Hi. Personally I would only put in the report the information the business needs to know. A colleague attended a tribunal recently and was asked why she had added info which had not been asked for. She was advised to only answer the questions asked on the referral. I would however discuss the history with the employee and signpost if necessary and document. You can also discuss with the individual and obtain their consent if they want anything added to the report.
