Subject: NHS Highland LD Service Review/ Community Service Models - Query
Dear All,
We are beginning an LD Service Review in NHS Highland to begin to address the many issues in our rather fragmented and hugely overloaded service.
As part of that review we are trying to think about a small group of complex challenging patients who have exhausted current community service models (single tenancies have been the model of choice on the whole since resettlement) and who are blocking inpatient beds with significant impact across the service.
My query is whether there are any good (or bad) examples of different models of community provision elsewhere in the country? That reduce placement breakdown, reduce recurrent admissions, improve quality of life and are better value for money.
We have been talking about a group community setting that has a rehab type focus and a more robust staffing struture.
With strong links with our own service.
I would very much appreciate if people could let me know if there are any examples that we could consider as we work through our service review.
