Reportedly the Open file dialog has been causing problems although this example is slightly different from those reports. We haven't actually been able to reproduce the bug here so can only go with reports... We've heard that updating to the latest MacOS (10.12.3) fixed this for several people.
Going forwards fslview is to be replaced by a new, feature equivalent (and more), tool called fsleyes. I believe a pre-release version is available. I will ask FSL support team to point you at that.
