Here is the one we have on our institutional repository - this is where our digital surrogates are held. https://clok.uclan.ac.uk/takedown.html This policy derived from JISC legal advice and is probably in need of a review.
Where we have digitised things which are a little 'unclear' in terms of ownership - we add a statement to each item inviting contact and offering apologies and takedown. These are things which we are now looking at to register as Orphan works, but in the past we took a 'risk management' approach with.

Subject: take down policies
Hi all,
The Lanchester Interactive Archive project at Coventry University intends to make digital images of over 21,000 items from the Frederick Lanchester engineering/car manufacturing collection available online, along with an archives catalogue for the contents.
As a result we are formulating a 'take down' policy on data protection, human rights and IPR issues that may arise from the contents of the material displayed online.
I have seen some policies on this, such as the guidelines used by the Wellcome Library for access to personal data and copyright clearance & take down. But would welcome suggestions or advice on policies that others may have.
