I wonder if it might be worth contacting someone within the Medical Illustration sector. As they deal with consent for photographing children all the time. This is their association website http://www.imi.org.uk/ they would be a good benchmark if you wish to consult more widely within the industry.
I have lots of contacts in that sector should you need them.

Hello, we are in the process of updating our consent for for photography of minors at events such as learning activities for children under 18 & vulnerable adults. The photography is carried out professional photographic studio team and consent is signed and authorized prior to any photography taken by the parent/carer.
I would be most appreciated if you could share your forms so that I can demonstrate a wider consultation but would also to gauge what level of permission you give ie as broad as social media & 3rd parties such as HLF or more restrictive.
