As I had so many people ask what I was did .... here we go.....I used the HSE tool and its brilliant ( hats off to whoever devised it!) It will work in any business trust me.
What I would say is to sell to employees I would call it a 'Health and Wellbeing audit' as it removes negativity.

I used it on a workforce of approx 160. I sent out the form with a covering page to explain it was anonymous and that I would compile the data so they felt safe they could answer honestly and actually encourage completion. As it was a HSE tool they trusted me and it as it was not skewed to meet anyone's needs.
It also stated that unless they ticked which area they worked in they would not be included in the results as I needed that to ensure I broke down the data for the managers to ensure it had value and was not a tick box exercise. I think this is important to employees.
We had nearly 70% returned and I then broke it down into 13 departments.
The tool will only will give data if you enter 10 or more into it.
As I am anal I then changed the results in the 'raw data' section into colours so all 5's were dark green, 4 light green, 3 yellow, 2 amber and 1 red to make it easier and more visual for the managers to spot trends.
I then set a lunch and learn with all managers to go through the whole workplace results to explain how it works and what it can do to support them in their own areas.
I then sent each manager their results anonymised . I suggested that managers looked at the raw data and decided which were the top 10 best responses and top 10 worst ( using the colour). THEN and only THEN look and see which questions they were ( so as not to skew which they chose!) I suggested that they then meet with all their staff and discuss the top 10's as a positive and negative and a way to look for consensus of approach to improve.
