I’d like to mention FMS, a smartphone-based travel diary app (and web interface) that has been applied in different contexts, including a survey with 1000+ people in Singapore. More recently, there was a happiness survey (https://happymobility.org/pages/home). It was developed by MIT.
One that may relate to Tom’s question is QualiT (also based on FMS), a survey about bus quality assessment, currently running in Boston (http://qualit.mit.edu).

It is a modular system, thus the background maps etc. would be easily
applied to other places.
The activity locations, trip purposes etc. are generated via combination of
map matching and machine learning (thus you may need to train the system
first if you want to use it in the UK).
We are also in the middle of making this app as an open-source property,
but is still an on-going process.
As for the app that prompt travellers real time and also can track
travellers' movement, we used sbNavi (SBOING.net<http://SBOING.net>) for measuring
door-to-door passenger travel satisfaction, as a part of METPEX (FP7
