You might find the article at http://research-data-toolkit.herts.ac.uk/2013/06/data-loss-in-the-ddud/ is a useful example of an RDM incident at the personal end of the scale rather than at an institutional level. It is an account of the experience of a group working in health research, although I did not identify it as such at the time of writing.
